This is a free access article, which means anyone can read the article free of charge. There is also an option for CEUs. SNF QRP Review and Correct Reports & Confidential Feedback Reports

SNF providers were recently notified of two issues affecting the SNF QRP Review and Correct Reports & Confidential Feedback Reports.

An issue was identified within the SNF Review and Correct Report, if one or more modification records are submitted for a patient stay, the most recent submitted assessment, which was accepted by the ASAP system on or prior to the submission deadline for Q1 and Q2 of CY2017, was not being used in the measure calculation.

All quality measure data has been recalculated and is now available in the SNF Quality Reporting Program report category in the CASPER Reporting application. Providers are encouraged to request the report to view updated measure results. The updated report should replace versions of the report requested prior to the issue notification.

The second issue that was identified in the technical coding, which affected our calculation of one measure contained in the SNF Review and Correct Report related to the measure Application of Percent of LTCH Patients with Admission and Discharge Functional Assessment and a Care Plan that Addresses Function, also affected the SNF QRP Confidential Feedback Report (SNF Quality Reporting Program (QRP) Quality Measure Report) is still being resolved and further guidance is forthcoming.

As a reminder, all data for assessment-based measures required in the SNF QRP in CY2017 will remain open to modifications until May 15, 2018. We anticipate that the data currently “frozen” as of August 15, 2017 to be subject to updates as of mid-September. Another notice will be issued once the Review and Correct Report Q1 2017 SNF QRP data is open again.

Further guidance and timelines has been posted to the SNF Quality Reporting Program Data Submission Deadlines webpage.

All facilities will receive an email blast notifying them of the above. We will continue to send follow-up announcements as further guidance can be given.

The Provider Meetings will continue to be held in two sessions, the first session will be for Residential Care Facilities/Assisted Living Facilities, and the second session will be for Skilled Nursing Facilities/Intermediate Care Facilities. This year in order to avoid repetition in the program we will conduct a combined session for all provider types to cover topics of interest relevant to all levels of care. The combined session will run from 10:15 until noon. Registration is not required. CEU’s will be available for Nursing Home Administrators.

Life Safety Code Reminder: The 2012 NFPA annual door inspections need to be completed by July 5, 2017

 

NFPA 80, 2010 edition:

 

3.3.95 Qualified Person. A person who, by possession of a recognized degree, certificate, professional standing, or skill, and who, by knowledge, training, and experience, has demonstrated the ability to deal with the subject matter, the work, or the project.

 

5.2.3.1 Functional testing of fire door and window assemblies shall be performed by individuals with knowledge and understanding of the operating components of the type of door being subject to testing.

 

5.2.3.2 Before testing, a visual inspection shall be performed to identify any damaged or missing parts that can create a hazard during testing or affect operation or resetting.

 

5.2.4 Swinging Doors with Builders Hardware or Fire Door Hardware.

5.2.4.1 Fire door assemblies shall be visually inspected from both sides to assess the overall condition of door assembly.

5.2.4.2 As a minimum, the following items shall be verified:

(1) No open holes or breaks exist in surfaces of either the door or frame.

(2) Glazing, vision light frames, and glazing beads are intact and securely fastened in place, if so equipped.

(3) The door, frame, hinges, hardware, and noncombustible threshold are secured, aligned, and in working order with no visible signs of damage.

(4) No parts are missing or broken.

(5) Door clearances do not exceed clearances listed in 4.8.4 and 6.3.1.7.

(6) The self-closing device is operational; that is, the active door completely closes when operated from the full open position.

(7) If a coordinator is installed, the inactive leaf closes before the active leaf.

(8) Latching hardware operates and secures the door when it is in the closed position.

(9) Auxiliary hardware items that interfere or prohibit operation are not installed on the door or frame.

(10) No field modifications to the door assembly have been performed that void the label.

(11) Gasketing and edge seals, where required, are inspected to verify their presence and integrity.

Healthcare-Associated Infections (HAIs) cut across all care settings – acute, ambulatory and long-term. All settings are currently (or soon will be) required to have an infection control and prevention program including an antimicrobial stewardship plan. These workshops will provide the latest evidence regarding the prevention of HAIs across settings with a strong focus on incorporating this evidence in clinical practice. Regulatory considerations and specific strategies to bridge the gap in coordinating care when patients transition from one setting to another will be included.

To better secure our applications, QIES security will require each user to successfully login every 60 days, effective June 26, 2017. If this does not occur, the account will be disabled and can only be re-enabled by contacting the QTSO Help Desk at (800)339-9313.

Accounts that have no activity for more than 365 days will be deleted. Once your account is deleted, you will not be able to use or re-enable the account. If your account is deleted, you will need to request access through CMS as you did originally; you will be assigned a new User ID and password when the access is granted.
