Pioneer Network’s Hot Topics: Culture Change in Action

January 23, 2019: Trauma-Informed Care: Resilience and Vulnerability
Guide: Dr. Susan Wehry, a geriatric psychiatrist, Chief of Geriatrics at the University of New England College of Osteopathic Medicine (UNE COM)

Adverse life events–including adverse childhood events (ACEs)–can affect us throughout our lifespan.  As the number of these events increases, so does the risk for negative health and diminished well-being. This is true for both caregivers and residents of long-term care. However, even in the face of trauma, people still possess an ability to return to being healthier and more hopeful, with less caregiver burnout.

At the end of this presentation, participants will be able to:

  1. Discuss the relationship between person-centered and trauma-informed care
  2. Define trauma, trauma-informed care, resilience and vulnerability
  3. Nurture resilience
  4. List resources for providing a trauma-informed approach to care

That’s why it’s critical for leaders to develop a plan to smooth the transition and promote financial health. Join us for a one-day workshop designed to help you implement PDPM, featuring deep-dive sessions specific to executives, clinicians and revenue cycle professionals.

This workshop is co-sponsored by BKD CPAs & Advisors, LeadingAge Missouri and the Missouri League for Nursing.

Empira will share successful strategies for creating a culture of sleep with consideration of all 24 hours in a day. This is Part 3 of a three part series. Join the presentation even if you missed Parts 1 and/or 2.

February 15, 2019: Fake Life and Real Life – Let’s Talk!
Guest: Sonya Barsness, Gerontologist

How much of what is offered to people living in long term care communities is fake?

Much is at stake with fake. Let’s call it out and consider why we think it’s okay. We’ll talk about examples, like using fake pets and babies, mock tails and near beer, the folding of fake towels and whether we are asking the right questions in thinking about if and when to use them. Let’s compare it to real and what each says about how we care for and treat older adults. We’ll be honest, our bias is to encourage all to move more in the direction of real life rather than fake. Let’s compare what our goals are when we are fake and contrast it with what we want for people – real living.

RAI Process from Start to Finish

January 22-23, 2019: Columbia
April 17-18, 2019: Kansas City
June 18-19, 2019: Chesterfield
August 6-7, 2019: Columbia
October 15-16, 2019: Springfield

This workshop will look at the RAI process from the beginning to the end. The MDS is used for both a clinical assessment and a financial assessment and this workshop will discuss the rules on scheduling these assessments and meeting the requirements individually and combined. From coding the MDS, we will then look at the CAA to care plan process.

This workshop is for individuals in long-term care who coordinate and/or code the MDS. Typically, this is an RN or LPN with the title of MDS Coordinator or Care Plan Coordinator. This is also for the DON or ADM who needs a better understanding of the complexity of the RAI process.

Medicare from Start to Finish

March 19, 2019: Columbia
May 21, 2019: Springfield
July 16, 2019: Kansas City
September 17, 2019: Chesterfield
November 13, 2019: Columbia

This one-day workshop will begin with how to gain access to and read the MDS reports from CASPER in order to self-audit your RAI process. The completion of the MDS is becoming more and more difficult and confusing. This workshop will help the MDS Coordinator and other members of the interdisciplinary team to understand the admitting criteria and the ongoing documentation needs of the Medicare resident. The RAI process in relationship to Medicare is also confusing and errors can be a financial nightmare for facilities. This workshop will help clear up the confusion surrounding the process.

This workshop is geared towards those individuals working in long-term care who have a leadership role such as the ADM, DON, ADON, or MDS Coordinator.

November 9, 2018: Know Better, Do Better – Part 4
Guest: Sarah Brown, Executive Director, Empira

Empira has been working to better understand the wants and needs of older adults, given the following truths:

  1. The primary reason for many behavioral expressions centers around the lack of support for “work of aging.”
  2. The ultimate success of inevitable aging is the ability to maintain integrity with a sense of closure and completeness and accept death without fear.

Based on these understandings, an approach called ResoLute (Resident Empowered Solutions on Living until the End) is currently in development to support conversations about life and mortality – to identify and uphold what matters most.

October 12, 2018: Know Better, Do Better, Part 3
Guest: Sarah Brown, E.D. Empira

The status quo had to change. Consider these facts:

  • Restorative sleep is vital to the overall well being of humans
  • Sleep fragmentation is a primary contributor to some of the most challenging issues facing long term care(e.g. falls, behavioral expression, pain, incontinence, skin breakdown, unnecessary use of anti-psychotics, function decline and the list goes on)

Empira recognized the challenge and is now making a “night and day” difference for the residents they serve through a culture of restorative sleep. For this, the 3rd show in our series, Sarah Brown will share what Empira has learned, how they transformed their culture and the impact of this on residents

Dementia Care Mapping

Dementia Care Mapping

October 15-18, 2018: Dementia Care Mapping for Realizing Person Centered Care
Location: The Neighborhoods by Tiger Place, Columbia

Dementia Care Mapping (DCM) is a method to evaluate the delivery of person centered care and, in general, to improve the quality of care delivered to persons with dementia in formal care settings. DCM goes to the real heart of caring by striving to assess care from the viewpoint of the person with dementia. It involves making and recording detailed observations. DCM provides valuable, clear and reliable information for those concerned with quality care for people with dementia. After the data is analyzed, immediate feedback can be given to care partners so that existing practices can be analyzed and improved.

Please see the flyer for more details including registration.

Downloadable Materials for Instructor-Led Training Hand in Hand: A Training Series for Nursing Homes Download

This training focuses on caring for residents with dementia and on preventing abuse. CMS, supported by a team of training developers and subject matter experts, created this training to address the need for nurse aides’ in-service training on these important topics.

The Hand in Hand: A Training Series for Nursing Homes was previously developed as an instructor-led training conducted by nursing home staff for nurse aides. The Hand in Hand training will now be available as a self-paced, online training and available to download for use as an instructor-led course.

Federal law requires that nurse aides complete in-service training on dementia management and resident abuse prevention. Additionally, facilities must provide dementia management and resident abuse prevention training to all facility staff, (direct and indirect care and auxiliary functions) contractors, and volunteers.

For help with registration and all other technical issues, please contact the ISTW Helpdesk at cmstraininghelp@hendall.com.
