Summary Strategies to Optimize the Supply of PPE during Shortages

CDC developed a quick reference table summarizes CDC’s strategies to optimize personal protective equipment (PPE) supplies in healthcare settings and provides links to CDC’s full guidance documents on optimizing supplies. These strategies offer a continuum of options using the framework of surge capacity when PPE supplies are stressed, running low, or absent.

Quality Reporting Program: Non-Compliance Letters for FY2021 APU

CMS is providing notifications to SNFs that were determined to be out of compliance with Quality Reporting Program (QRP) requirements for CY 2019, which will affect their FY 2021 Annual Payment Update (APU). Non-compliance notifications are being distributed by the Medicare Administrative Contractors (MACs) and were also placed into facilities’ CASPER folders in QIES on July 13, 2020. Either notification is official notice of non-compliance. Facilities that receive a letter of non-compliance may submit a request for reconsideration to CMS via email no later than 11:59 pm, August 18, 2020.

Reminder – Community Testing Opportunities, open to all Missourians

Missouri continues to announce community testing events across the state. Several communities will be conducting COVID-19 testing over the next two weeks. For these testing events, participants will not be required to have a doctor’s authorization and there will be no charge to participants. Anyone who wants to be tested may participate – you will not be required to have a known exposure or be experiencing symptoms. Everyone can register for this event, including those who may be experiencing symptoms and have struggled to be tested. Many of these testing events do not require you to be a resident of the county conducting testing. You must, however, be a resident of Missouri.

Missouri Guidance on Reopening of Long-Term Care Facilities

The guidance on reopening of long-term care facilities has been finalized. The guidance will be posted to the DHSS website at https://health.mo.gov/living/healthcondiseases/communicable/novel-coronavirus/professionals.php under Long-Term Care Facilities.

The department has contracted with the University of Missouri-Sinclair School of Nursing QIPMO team to assist homes through the reopening process. DHSS requests that homes contact the QIPMO team as they begin the reopening process.

The “usual” QIPMO team of nurses and leadership coaches will be available for assistance, and a couple of new team members have been added to the new CAT group – the COVID Accountability Team. Please, please feel free to directly contact your “representative”.

Nursing Home Five Star Quality Rating System updates, Nursing Home Staff Counts, Frequently Asked Questions, and Access to Ombudsman (REVISED)

  • CMS is committed to taking critical steps to ensure America’s nursing homes are prepared to respond to the threat of the COVID-19.
  • Nursing Home Compare website & Nursing Home Five Star Quality Rating System: We are announcing that the inspection domain will be held constant temporarily due to the prioritization and suspension of certain surveys, to ensure the rating system reflects fair information for consumers.
  • Posting of surveys: CMS will post a list of the surveys conducted after the prioritization of certain surveys, and their findings, through a link on the Nursing Home Compare website.
  • Nursing Home Staff: CMS is publishing a list of the average number of nursing and total staff that work onsite in each nursing home, each day. This information can be used to help direct adequate personal protective equipment (PPE) and testing to nursing homes.
  • Access to Ombudsman: We are reminding facilities that providing ombudsman access to residents is required per 42 CFR § 483.10(f)(4)(i) and per the Coronavirus Aid, Relief, and Economic Security Act (CARES Act).

Be Infection Control Survey Ready: Join the Health Quality Innovation Network’s Office Hours Series

The Health Quality Innovation Network (HQIN), the Quality Improvement Organization (QIO) for Missouri, is hosting an office hours series to support survey readiness and help nursing homes be responsive to identified opportunities, survey deficiencies and CMS enhanced enforcement actions. Each 30-minute session will demonstrate how to apply quality improvement principles to your infection control and prevention program and provide turnkey resources and tools to address gaps and drive improvement in your facility. A panel of nurses, infection preventionists and quality improvement professionals also will be available to answer questions.

The series begins July 14, 2020 and will cover the following topics:

July 14, 2020: Root Cause Analysis (RCA)
Facilities have worked hard to assess their infection control programs. But do you know the reason for the gaps or breakdowns in your program? We will share the HQIN QAPI RCA resource, which compliments the CMS QAPI RCA Guide.

July 28, 2020: Infection Prevention Action Plans
Once you complete the RCA process, then you must develop a plan of action. We will share the HQIN Infection Prevention Action Plan Templates. These templates compliment the CDC Infection Control Assessment tool. We will provide you with a template for key categories of improvement, i.e., hand hygiene, surveillance, environmental hygiene, etc.

August 11, 2020: Data Collection and Analysis
Once your plan is implemented, how do you know if it’s working? HQIN will share data collection tools to help you track and trend your improvement related to hand hygiene, terminal cleaning and PPE.

August 25, 2020: QAPI: Everyone Has a Role and a Responsibility
It takes a team to ensure improvement is made and sustained. HQIN will discuss how to engage your team in the quality improvement process in order to hardwire systems, change behavior, reduce gaps and to improve care and compliance.

New IDR Contract

The Department of Health and Senior Service has contracted with Healthcare Quality Innovators (HQI) to offer IDR services to Missouri Long Term Care Communities. HQI is Missouri’s designated Medicare Quality Improvement Organization and is based in Richmond, Virginia. HQI staff responsible for the IDR process are familiar faces, as they are the same staff that worked under the previous contract. HQI has begun contacting those communities with pending IDR requests. All new IDR requests will be submitted to HQI.

Missouri PPE Manufacturers

Below is a list of Missouri companies that are producing PPE, including what type of PPE (face shields, hand sanitizer, gowns, surgical masks) and their contact information.  Please feel free to reach out to these companies to see if you can develop a new supplier relationship.  Additional companies will be added as more companies acquire the capabilities to produce PPE.  The state will be providing some grant support to manufacturers so that additional types of PPE can be manufactured in Missouri, like N-95 masks and gloves.

Updates to the COVID-19 Module for LTCFs

In efforts to continue support of the nation’s COVID-19 response, the Centers for Disease Control and Prevention’s (CDC’s) National Healthcare Safety Network (NHSN) is excited to announce the following updates to the COVID-19 Module for Long-Term Care Facilities (LTCF):

  1. An NHSN COVID-19 Module Dashboard for LTCFs is now available for participating groups and The dashboard provides a summary of data entered in the COVID-19 Module and includes an interactive chart display where a user may customize data views. This first edition of the dashboard is concentrated on the Resident Impact and Facility Capacity pathway. However, we’ve already started working on the next edition, which will continue additional pathways and analysis options!! Guidance documents for groups and facilities to assist with navigating and understanding the features are available on the NHSN LTCF COVID-19 Web-page.

 

  1. You asked, and we listened! We recognize that many SNFs are setting up COVID-19 units to receive transfers from other LTCFs, such as Assisted Living Residences. These transfers result in increased demand on the receiving LTCFs. Therefore, the count for “Admissions” has been revised to include admissions and readmissions of residents who were previously diagnosed with COVID-19 from another facility. For additional information, please review the revised data collection form and accompanying instructions for the Resident Impact and Facility Capacity Note: LTCFs may update previously entered “Admissions” counts, but this is not a requirement.

 

  1. We aim to improve the quality of the COVID-19 data being reported. Users will notice Pop-up alerts as a reminder of potential data entry errors. For example, if a user attempts to enter more COVID-19 Deaths than Total Deaths, a pop-up alert will appear to remind user of the definitions. As a reminder, “COVID-19 Deaths” counts must be included in the Total Deaths count since Total Deaths is defined as the number of residents who died for any reason in the LTCF or another location since the last time “Total Deaths” counts were collected for reporting in the NHSN COVID-19 Module. For both counts, only include the NEW deaths since the last time the counts were collected for reporting in the Module, as the intent is to capture incidence.

 

  1. Non-modifiable fields have been added to each of the four pathway screens. Users will notice that each pathway now has three additional pieces of information displayed at the top of each pathway: Date Created, Facility CCN, and Facility Type.

The purpose of Date Created is to display the first date and time of data entry (manual or CSV file upload) for a selected calendar date and pathway. The date and time will automatically save and cannot be modified by the user.

The Facility CCN and Facility Type will also populate for each facility, allowing users to quickly verify the information associated with the data being submitted to CMS. These two variables may be edited by a facility user with NHSN Administrative rights; however, edits must be done outside of the COVID-19 Module by accessing Facility Info on the left navigation panel. CDC Guidance for Routine Off-Site Appointments

Division of Community and Public Health (DCPH) recently reached out to the CDC for guidance regarding long-term care facility (LTCF) residents attending routine off-site appointments and received the following response:

CDC does not recommend LTCF residents who go off-site for routine appointments (such as hemodialysis or other routine appointments or treatments) be kept in observation, in part because the observation period could be permanent for residents who have to leave more than once every two weeks. Additionally, LTCF residents who leave and return for appointments should not be considered new admissions or be housed in a COVID-19 care unit in the LTCF when they return. Placing LTCF residents who return in the observation or COVID area within the facility could put them at higher risk of exposure to COVID-19. While not a CDC recommendation, some LTCFs have opted to manage residents who go off-site for routine appointments as potentially exposed and use full Transmission-Based Precautions and/or have cohorted these residents together into the same area of the building. In these examples, it is very important to ensure these residents, who may be at increased risk of acquiring COVID-19, are NOT being placed into a dedicated COVID-19 unit or with others who are symptomatic/have increased risk of having COVID-19. Doing so would increase the risk of exposing these residents who go off-site to others at even higher risk of COVID. We have also heard about significant challenges with moving residents repeatedly and recognize that determining exposures can be challenging. Additional recommendations: Residents going off-site for dialysis or other appointments should wear a cloth face covering (or facemask) for source control when they leave the LTCF and ideally in the LTCF if they are leaving their room. Regular communication between the dialysis facilities/medical offices where appointments occur and the LTCFs (in both directions) is essential to help identify patients with fever or symptoms consistent with COVID-19 before they enter the facilities and facilitate resident care. Social distancing measures should be implemented, such as minimizing movement of these residents within the facility as much as possible. LTCFs should comply with the infection prevention and control guidance for nursing homes and LTCFs.

Changes to Staffing Information and Quality Measures Posted on the Nursing Home Compare Website and Five Star Quality Rating System due to the COVID-19 Public Health Emergency

The Centers for Medicare & Medicaid Services (CMS) is committed to transparency about changes in publicly reported information on nursing homes during the COVID-19 public health emergency.

Changes to the Nursing Home Compare Website and Five Star Quality Rating System:

  • Staffing Measures and Ratings Domain: On July 29, 2020, Staffing measures and star ratings will be held constant, and based on data submitted for Calendar Quarter 4 2019.
    • Also, CMS is ending the waiver of the requirement for nursing homes to submit staffing data through the Payroll-Based Journal System. Nursing homes must submit data for Calendar Quarter 2 by August 14, 2020.
  • Quality Measures: On July 29, 2020, quality measures based on a data collection period ending December 31, 2019 will be held constant.

SNFS are expected to report their Quality Reporting Program (QRP) data starting Quarter 3, which begins July 1, 2020

The March 27, 2020 Medicare Learning Network Newsletter (MLN) Exceptions and Extensions for QRP Requirements that includes SNFs applies only to Quarter 4 of 2019 (October 1-December 31, 2019) and Quarters 1 and 2 of 2020 (January 1-June 30, 2020). Providers are expected to report data and meet the QRP requirements beginning with Quarter 3, 2020 that starts July 1, 2020.

As stated in that March 27, 2020 MLN Newsletter, “In some instances, these exceptions and extensions are granted because the data collected may be greatly impacted by the response to COVID-19 and therefore should not be considered in the quality reporting program. CMS is closely monitoring the situation for potential adjustments and will update exception lists, exempted reporting periods, and submission deadlines accordingly as events occur.”

Starting with Quarter 3 that begins July 1, 2020, CMS expects providers to report their quality data. CMS will analyze the data recognizing that the COVID-19 public health emergency (PHE) remains in effect and could impact the quality data submitted. CMS will closely monitor the situation for public reporting of the data and provide any updates.

Updated Guidance on Invoicing Process for Testing and SNF COVID-Related Expenses

The guidance has been updated to request facilities bill Medicare first, if applicable, for testing costs and to provide contact information when submitting invoices. In addition, for SNFs only, updates have been made to the attestation form and information regarding costs being capped per facility.

Heat Detector Recall FYI

Edwards Recalls Mechanical Heat Detectors Due to Failure to Alert to Fire. This recall involves mechanical heat detectors used indoors to detect elevated temperatures as part of professionally installed fire detection systems for residential homes and businesses.

Mental Health and Wellness Resources

 

  • Mental Health First Aid Training – Mental Health First Aid is now available in a variety of formats, including a fully virtual course. Register today to be the one to make a difference for your friends, family members, colleagues, students, or children. This training is designed for the general community. This course is valued at $170 per person, but is being offered free through the Missouri Department of Mental Health. The flyer will be distributed around the 15th of each month with additional trainings. Check out http://mhfamissouri.org/events/ for more frequent updates.

Mental Health First Aid teaches you how to identify, understand and respond to signs of mental illness and substance use disorders. This training gives you the skills you need to reach out and provide initial support to someone who may be developing a mental health or substance use problem and help connect them to the appropriate care. Youth MHFA is also available for adults working with youth (age 12-18).

 

  • DMH Office of Disaster Services is hosting two virtual trainings given by Dr. Patricia Watson on Stress First Aid. Below you will find the details on the two courses.     • Stress First Aid for Self-Care and Coworker Support
      Presented July 13th (1:00 pm – 5:00 pm CST)
      This four (4) hour course delivered via webinar, includes a set of supportive actions designed to help those who work in high-stress jobs reduce the negative impacts of stress. SFA recognizes that critical incidents are not the only stressors that professionals face and is therefore based on the acknowledgement that stress reactions can be ongoing and cumulative, resulting from multiple sources. It is designed to be attentive to traumatic events but also on work and personal stress, as well as loss, and inner conflict or moral injury.
    • Stress First Aid for Use with Patients, Clients and Customers
      Presented July 16th (1:00 pm – 5:00 pm CST)
      This four (4) hour course delivered via webinar, includes a set of tools that helps workers support and reduce stress in those they serve in public. This version of SFA is similar to Psychological First Aid (PFA) in that it aims to reduce distress and help move people towards more effective coping. However, it has a broader focus then PFA, in that it is intended to be used with those who are significantly stressed for any reason, and at any point in their exposure to stress.

 

  • Train the Trainer for Psychological First Aide (PFA)

Psychological First Aid is an evidence-informed modular approach to help people of all ages in the immediate aftermath of disaster and terrorism (and traumatic events). Individuals affected by a disaster or traumatic incident, whether survivors, witnesses, or responders to such events, may struggle with or face new challenges following the event. PFA was developed by the National Child Traumatic Stress Network and the National Center for PTSD, with contributions from individuals involved in disaster research and response.
