March 2, 2016:  Interdisciplinary Approach to Continence Improvement
Presenter:  Kathleen Weissberg from Select Rehabilitation
All health care disciplines have a role in skilled intervention to treat continence.  This webinar provides an overview of anatomy and physiology of normal voiding as well as normal function related to continence.  Different types of incontinence are identified and assessment/treatment strategies are offered for each.  This course takes an interdisciplinary approach to continence improvement and demonstrates roles of each team member.  Emphasis is placed on the role of nursing in completing an assessment of voiding, identifying triggers and bladder stimulants, voiding habits and patterns, and implementing individualized care approaches.  Restorative and compensatory strategies for continence management are reviewed as well as the unique role of physical/occupational therapy in this program.  Documentation and care planning related to behavioral aspects of continence improvement are reviewed.

This workshop will look at the RAI process from beginning to end.  The MDS is used for both a clinical assessment and a financial assessment and this workshop will discuss the rules on scheduling these assessments and meeting the requirements individually and combined. Item-by-item coding will be reviewed.  From coding the MDS, we will then look at the CAA to care plan process.

This one day workshop will begin with how to gain access to and read the MDS reports from CASPER in order to self-audit your RAI process. The completion of the MDS is becoming more and more difficult and confusing. This workshop will help the MDS coordinator and other members of the interdisciplinary team to understand the admitting criteria and the ongoing documentation needs of the Medicare resident. The RAI process in relationship to Medicare is also confusing and errors can be a financial nightmare for facilities. This workshop will help clear up the confusion surrounding this process.

CMS Memo S&C 16-04-NH: Focused Dementia Care Survey Tools

In response to feedback from stakeholders and partners of the National Partnership to Improve Dementia Care in Nursing Homes, CMS is sharing the revised survey materials that were developed for the 2014 Focused Dementia Care Survey Pilot and 2015 expansion effort. The intent is that facilities would use these tools to assess their own practices in providing resident care.

2015 State of the Art Person-Centered Care Survey

It has been almost a decade since the last national survey explored how care communities are moving away from more traditional systems of care and adopting individualized care practices.  Whether it is a change in how meals are served, daily routines, or an enhanced physical environment, the momentum for change is growing.  Even CMS is looking to embed Person-Centered Care much more deeply in their regulations.

Nursing homes that offer short and/or long term care and assisted living communities are invited to participate in the 2015 State of the Art of Person-Centered Care Benchmark Study.

Updated MDS Information – additional

On October 1, 2015 a new version of the RAI Manual became effective, Version 1.13.  Most of the changes were minor but there is one important change to point out.  Prior to the update, Significant Change MDS’s were required if a resident enrolled in or discontinued hospice.  Those rules are still in effect, but now, a Significant Change MDS is also required when a resident changes hospice providers and remains a resident at the nursing home.  This new rule is in the RAI manual in Chapter 2 on page 21.  A Significant Change MDS may also be required in situations when the hospice providing services is purchased by another hospice provider.
