Minimum Data Set Converting To Verizon – The AT&T connection for transmitting the Minimum Data Set (MDS) will be disabled the first week of January 2012. All providers must convert to and transmit via Verizon. This requires successful installation of Verizon. The new Verizon phone number for providers to call for assistance is 888-238-2122.
