CMS sent revalidation notices to enrolled skilled nursing facilities (SNFs) from October – December 2024 to collect data on ownership, managerial, and related party information – https://www.cms.gov/medicare/enrollment-renewal/providers-suppliers/chain-ownership-system-pecos.
ALL SNFs have until May 1, 2025, to submit their revalidation. You must report this data on the revised Medicare Provider Enrollment Application Form (CMS-855A). Please note that CMS will not be updating the online Medicare Revalidation List to include the SNF off-cycle revalidation schedule.
