Early identification and communication of residents’ changes in status are essential to avoiding hospital transfers. Reducing avoidable rehospitalizations are of growing interest to hospitals and may become criteria for future referrals. INTERACT II focuses on the management of acute change in resident condition. It includes clinical and educational tools and strategies for use in every day practice in long-term care facilities.
