Providers in Arkansas, Missouri, Oklahoma, Puerto Rico and Texas are invited to participate in this clinical pharmacy sharing series to improve care for Medicare patients by learning about the following:

  • Improving medication reconciliation among providers
  • Reducing adverse drug events and polypharmacy
  • Reducing unnecessary hospital admissions and readmissions
  • Improving medication adherence
  • Best practices, lessons learned and evidence-based guidelines from other providers in the region

 

About the Presenter:  Niki Carver, PharmD, CPPS, and Pharmacy Consultant

Dr. Carver is currently the pharmacy director at Arkansas Continued Care Hospital (ARCCH) of Jonesboro, a 44-bed long-term acute care hospital. In addition to her role with ARCCH, she serves as pharmacy consultant for the TMF Quality Improvement Network. In this role, she is a valuable resource for medication safety initiatives and care transitions.
